We are a collaboration of experienced physicians and providers that have joined forces to offer our unique services to the community under one roof.

All of us operate our businesses separately. AND you can chose to see any of us individually and/or choose one of our membership plans.

What your

membership includes :

1 visit / month flexibility to see any of our wellness center providers. You choose the service to meet your wellness needs. All of our providers are experienced & will be able to contribute to helping you age gracefully & design a longterm healthy 401K plan.

What if I desire more than one monthly visit?

The monthly well care membership plans give you one visit monthly to see any of the participating well care providers. If you desire an extra visit you can schedule directly and purchase that with any of our providers.

Is primary care included in my membership?

Yes, under the Direct Primary Care PCP membership

you will have a dedicated PCP.

This plan is currently just $95/month

What if I have a health issue I need

serious help with?

If you're on medications you'd rather not be or other providers have been unsuccessful in improving your health, choose Dr. Croft's Virtual Exclusive Wellness Plan designed specifically for those needing intensive functional and restorative medical needs. OR see any of our providers websites for specific care packages.

The well care plan is for wellness and not serious health issues needing more attention or chronic disease management.

I'd like both primary care and wellness options.

To have both a PCP and access to our expert wellness providers, you will need to enroll in both the

DPC plan $95/mo. and the well care plan $150/mo.

Insurance ?

Russ Howard, APRN is the only provider who bills any insurance carrier.
We expect full payment at the time of services rendered OR pre-purchase of any care package offered.
You can self submit an insurance claim with help from your insurer. We cannot guarantee any amount of reimbursement for any services or labs/studies ordered that you agree to have done. You are responsible to understand how your insurance works.

The Annual Service Fee

Your modest annual service fee, which equates to ~$18/month, helps cover all administrative aspects of operating your membership plan.

It is paid upon enrollment and annually the month you joined.

It is not prorated or refundable.

Children are free after two (2) enrolled individuals.

And a family will only ever pay a service fee for a max of two (2) enrolled members.

How do I get started?

Choose the specific care plan that is right for you and complete enrollment.

How do I schedule visits with providers?

New Dimensions Wellness Center has a central scheduler. Once you enroll in a specific plan or contact us at info.newdimensionswellness@gmail.com you will be added to the scheduler. Then you will be able to see the available providers and schedule directly into their appointments.
